I hereby waive all rights to ownership of any photographs, slides, video, and audio in which I appear, including any right that I may have to inspect and/or approve the finished product or the editorial layout that may be used in connection therewith, or the use to which it may be applied.

I am at least 18 years of age and have the right to contract in my own name. I have read the above and fully understand the contents. This release shall be binding upon me and my heirs and legal representatives.
