Nomination Form

The DWC Alumni Association seeks to honor DWC’s finest graduates. Nominees for this award are recognized for their service to the college and/ or for their contribution to their profession or the community at a national or international level. They must hold a degree from DWC and be living at the time of the nomination.

If you wish to submit a nomination, please complete the following form and list the most outstanding contributions to Daniel Webster College, profession or community made by the nominee.

In addition, you may also mail in any material that would be helpful to the selection committee such as a resume, newspaper clippings, lists of prior awards, publication, etc. However, please limit your supporting data to one 9’ x 11’ envelope.

All submissions must be postmarked or received electronically by the deadline for nominations of 3:00 p.m. on December 31st. The selection committee will present their findings to the Alumni Association Board for a vote at the winter meeting.
